Thus far we've been unable to verify or reproduce this bug. Or, it has been observed but it cannot be triggered with a reliable test case. You can help us out by editing your report or adding a comment with more information.
Descriptive Problem Summary:

This is hard to explain, my demo will have to do most of the explaining. The bug is, that an icon once added to overlays, cannot be removed using the same method sometimes.

Numbered Steps to Reproduce Problem:
Download File2Text.zip from http://mediafire.com/gsus7125 and look at it's code, and use it.

Does the problem occur:
Every time? Or how often? Randomly, sometimes it decides it'll work a few times then it'll just 'bug up' and stop working.
In other games? Idk
In other user accounts? Yes
On other computers? Yes

When does the problem NOT occur?
Never

Did the problem NOT occur in any earlier versions? If so, what was the last version that worked? (Visit http://www.byond.com/download/build to download old versions for testing.)

It has been this way in all previous versions that I am familiar with, which isn't very many. Maybe back to 460

Workarounds:

None
I was not able to verify any issue in your test project. Removing the overlay worked just fine.

It doesn't happen 100% of the time, it's about 20% of the time, if you swap between Add_Tail and Remove_Tail (Or whatever the verbs were called) about 20 times you should see it eventually happen.

Maybe it has something to do with certain computers only? But it's a real bug, I can use that exact same source listed in the bug report and get the bug to happen in less than 30 seconds.

There is no runtime error associated with the bug.

I will come back here with video evidence soon.
Ok I now have video evidence. Go to http://mediafire.com/gsus7125 and download Bug.avi. It's a 100 mb file but I had to have it that large to maintain quality so you can see what's going on and such.

I couldn't upload it to youtube because the quality reduction would mean you can't see some important things.

Watch it all the way to the end because the end is where the most prominent example of this bug happens.
I see it happening in your video, but there's still no consistent way to get it to happen. Unless I can reproduce this I can't fix it.
