Open Source, but can I still keep the credit?

I want to maintain the credit for the original work I've done in a C#, window's form based program I've created, specifically, I'd like to keep a little "About" button in my program with the text that says I created it and you can email me for questions.

Is there an open source website that would allow me to distribute the source for modification so long as that button and text is kept in tact?

What about an open source website that allows users to incorporate the Creative Commons licenses?

=( Creative Commons isn't really a very good software license. I'd recommend a GPL (http://www.gnu.org/licenses/) then just note that would expect to keep some credit in the About window of any resulting program.

Again, sourceforge would be the place to go for this.
