Game Design & Organization in Design Philosophy

How do you all go about it? I'm more of a visuals person with jotted down, scattered notes: unorganized. I feel that helps with eating away at my motivation when doing artwork and designing a game. I'm almost certain that if I had better organizational skills, things would get done much faster than they're currently going.
Any tips?
**I've also never been too keen on completely writing out a design document, is there an simplistic method to writing one out?
