This issue has been resolved.
1. My opponent controls Mammoth Graveyard and enters his battle phase
2. I activate Ultimate Offering and pay 500 to summon Hitotsu-Me Giant
3. The Yugi bot breaks and stays in the battle phase and does nothing, only to lose when the time runs out.

Can you tell me what step you activated ultimate offering?

A good way to know is what the text said when you were asked to respond.

"Respond to battle phase - start step"
"Respond to xxx attack declaration"
"Respond to xxx attacking yyy"
"Start a chain"

It would have been something like one of the above.

I am also having trouble reproducing this bug.

It was respond the battle phase - start step. As soon as he entered the battle phase, I activated it. If that wasn't the step it activated on, the only thing I'm 100% sure is he DID NOT DECLARE an attack when I activated. Hell, he didn't even start it.
